To obtain services from the MAB-Mackay Rehabilitation Centre,
clients must have received a diagnosis from a healthcare
professional in one of the following areas:
Motor Impairment: Children between 0 and 18 years of age (18 to
21 years of age if still attending high school)
Communication Disorder: Children between 0 and 18 years of age
(18 to 21 years of age if still attending high school)
Deaf and Hard-of-Hearing: All ages
Vision Impairment: All ages
The referral source must include all reports documenting the
diagnosis, as well as any other pertinent medical or professional
report (Occupational Therapy, Physiotherapy, Psychology, Optometry,
etc.), with the Request for Services Form (DSIE Form for
professionals working with the Health and Social Services
Network).
The following Request for Services must be opened in Read Only.
You then have 2 options:
print the document, complete it by hand and mail it to the
following address:

complete the document at the computer, save it in your files
under a different name and forward it to us as indicated above.In
either case, it is important that the Request for Services be
accompanied by all mandatory reports.
In order to facilitate the exchange of information between the
MAB-Mackay Rehabilitation Centre and other interveners involved in
your file, please print the following Authorization to Release
Information in the User’s File, fill it out (the date and your
signature are very important) and mail the original to us at the
above address.
