Clientele
Adults and seniors, 18 years old and over, with a combination of
hearing and vision loss.
Services offered:
Assessment of auditory and communication skills
Assessment of functional vision, visual skills training and
ocular health assessment as determined by a specialized optometric
evaluation and equipment.
Psychosocial support and counseling for the client and
significant other
Counselling and training in strategies for improved
communication with family and surroundings
Assessment and training for autonomy in activities of daily
living
Assessment and training in orientation and mobility
Assessment and training in communication skills
Assessment and training in the use of technical aids and
adapted computer aids
Assessment, counselling and training in the use of technical
aids for the Deaf and Hard-of-hearing
Complete before and after cochlear implant rehabilitation
program which includes:
Complete Intensive Functional Rehabilitation (IFR) Program
after a cochlear implant
Technical support for cochlear implant (Troubleshooting and
emergency service: 514-488-5552, ext. 4016)
Support for integration into the community, work and leisure
activities
A multidisciplinary team of professionals working in
collaboration to respond to the needs identified by the
client/parents from the following disciplines:
Audiology
Optometry
Orientation and Mobility
Social Work
Special Care Counselling
Speech & Language Pathology
Vision Rehabilitation

To access services:
The person must be diagnosed with a sensorineural or permanent
conductive hearing loss with at least 25 decibels of loss in the
better ear.
Eligibility for the RAMQ funding program for aids and devices
requires that a client have a visual acuity in the better eye after
correction of <20/70 or <6/21, or whose visual field in each
eye is less than 60° in the 180° or 90° meridians after correction
with the appropriate ophthalmic lenses.
The referral source must include a copy of a recent eye report
(<1 year) as well as any other pertinent medical and
professional reports (Occupational Therapy, Physiotherapy,
Psychology, Optometry, etc.) along with the Request for Services
form (DSIE) for professionals working within the Ministry of Health
and Social Services Network or the inter-establishment referral
form.
All documents must be sent to Admissions.
Referral for
services can be made by the individual, the family or a member of
the community.
