impairment in two or more areas: motor, visual and/or auditory.

Parent group information and support during the entire
rehabilitation process
Access to the Dysphagia and Feeding Disturbance, Orthopedic,
Saliva Management and Low Vision clinics
A multidisciplinary team of professionals working in
collaboration to respond to the needs identified by the
client/parents from the following disciplines:

Medicine (developmental paediatrics; Ear, Nose and Throat
(ENT); orthopaedics)

Physiotherapy

To access services:
To be eligible for services, a child must be diagnosed with
multiple impairments (motor, auditory and/or visual).
The referral source must include a copy of the following
reports according to the type of impairment:
Sensorineural or permanent conductive hearing loss: Copy of the
audiogram
Motor disability: Diagnostic medical report
Visual impairment: Diagnostic ophthalmological or optometry
report
All documents must be sent to Admissions along with the Request
for Services Form (DSIE Form for professionals working within the
Health and Social Services Network).
